Background
Sing for Life Breast Cancer Foundation of Arizona (SFLBCFA) is a nonprofit Arizona based organization founded by a breast cancer survivor and professional singer, Laura Fial. Laura gained support throughout her diagnosis by surrounding herself with others afflicted with breast cancer. In 2001 a choir of breast cancer survivors known as "The Soul Survivors" was born. "The Soul Survivors" perform for free at a variety of fundraisers in the greater Phoenix area. Funds generated from CD sales and sponsorships offset production costs. Once these expenses are met, the foundation annually awards awards a scholarship to an outstanding student with aspirations of assisting cancer patients and their families. Initially, the name of the organization was Breast Cancer CD2000, but in 2006 the name was changed to Sing for Life Breast Cancer Foundation of Arizona, to better reflect the organization’s philosophy and mission statement.
SFLBCA, Inc. awards at least one annual scholarship in the name of “The Soul Survivors” for a minimum of $1,000.00, beginning October 2004. The scholarship is designed to fulfill a personal educational need or project to assist cancer patients and their families.
Eligibility
Eligible applicants must be a matriculating student or graduate of an accredited undergraduate university or nursing/therapy program in the United States. They must have received or are currently working toward a bachelor’s degree. Applicants must demonstrate an interest in oncology, an interest in breast cancer is preferred, but applicants with interests in other aspects of oncology also will be considered.
Relatives of SFLBCFA, Inc., members or its Board of Directors or Soul Survivor group are not eligible for this scholarship.
Approved Uses
The scholarship award must be used for tuition, continued education, books or specific equipment to fulfill a need or facilitate a project. It is not to be used for rent, lodging, seminars, or costs associated with certification.
Selection Criteria
All applicants must complete an application form along with a type-written essay describing their aspirations to help cancer patients and how the scholarship will help them achieve their specific goal. Applicants are also required to submit a song or poem that expresses their feelings about helping and supporting others.
The Board of Directors reviews all applications and awards the scholarship to the applicant that it feels will best fulfill the intended purpose of the scholarship fund. Although only one scholarship is awarded per annum, the Board, at its sole discretion, may elect to award additional scholarships to applicants that it deems to be highly worthy of such an award.
The Board reserves the right to designate a third party organization to administer and award the scholarship, depending on the number of applications received.
Scholarships are awarded purely on the basis of merit, without regard to race, gender, socio-economic status, or disability.
Application Deadlines
Applicants are invited to submit completed applications beginning October 1 of each year. There is a 6-month period to complete applications. The deadline for completed applications is April 1 of the next calendar year. Applications will be reviewed and the award(s) announced in October, at a fund-raising event sponsored by the Foundation.
Scholarship Amount
An amount of $1,000.00 will be awarded to each scholarship recipient.
Obligations of Scholarship Recipients
In return for accepting the award, recipients who live locally in Arizona are required to make themselves available for public appearances during the year in which they receive the award. This would include fundraising events, CD release parties, or other events sponsored by the Foundation. One of these appearances will be at an annual fundraising event in the spring to speak and/or present a new award to the next recipient. At this time they will also submit a letter evaluating the progress they made during the course of the year using the funds they received.
